Our staff is trained for public transport and the driver is unable to assist with mobility impairments during transfers. If you use a wheelchair you must use the wheelchair lift. Participants are encouraged to travel with a personal assistant who is capable of assisting transfers if assistance is needed. The AGNES is the primary intake and ongoing assessment form for the Wyoming Department of Health's services for seniors. It is required by WDH that this intake form is filled out annually. Without the form the KSC is unable to report you as a participant. This directly impacts future grant funding.
